Medical records are written for patient care, not litigation. They can be repetitive, incomplete, contradictory, and difficult to interpret without understanding how healthcare actually works.
AI may identify a diagnosis, medication change, imaging finding, treatment gap, or symptom change. Identifying the information and understanding its significance are different.
A diagnosis repeated throughout a chart may have been copied forward. An imaging finding may be chronic or incidental rather than related to the injury at issue. A medication change may signal worsening symptoms. A treatment gap may look concerning until another part of the record provides the explanation.
Our nurses are not simply asking, “What does the record say?” We are asking: Does it make clinical sense? What changed? What may be missing? What deserves a closer look? What does the attorney need to know?
